After Care Follow Up Assessments

This set of surveys are designed to monitor clients’ sociological and emotional progresses for follow up or potential intervention.

A variance between challenge ratings and after care ratings indicates a greater risk when the variance is greater than 10%.
A warning message is sent to the system administrator for follow up or potential intervention.

Such assessment spans over the first year of treatment that starts once patients are able to participate in the process. The first time this type of survey is conducted establishes a patient’s baseline. The first batch of self-assessment surveys is conducted by BRA or its designee for follow up or potential intervention.

Self-assessment tests can be conducted as many times as determined by clinicians. However, some milestones (30 days, 60 days, 90 days, 180 days, 270 days & 365 days) are however mandatory to maintain statistical integrity to the system administrator for follow up or potential intervention.
